Go to the "Common Forms" section of the hazardous waste declaration and management information system (IWR&MS) and download the application form for online password retrieval. Fill out the form and affix your company's seal, and attach relevant supporting documents such as the factory registration certificate, company registration documents, cleaning permit, processing permit, etc. Fax the application to your local environmental protection agency and confirm receipt of the fax within 5-10 minutes. The processing time is approximately 1-3 business days. The environmental protection agency will contact the applicant to provide the password.
